Who We Are

Elevate Community Services was founded by Shalom Safari, MSN, RN, a healthcare professional experienced in nursing leadership and community-based behavioral health services. He created Elevate to provide compassionate, practical support that helps individuals and families build skills, access resources, and achieve greater independence.

Our mission is to empower individuals and strengthen families through compassionate, person-centered services that promote stability, independence, and meaningful community involvement.
